Used to Record All Formal Complaints. This is the form you must complete to file a formal complaint with the CBA regarding a Member of this organization.Nature of ComplaintComplaint must be in writing with the following reason(s)- Breach of Code of Ethics – name the part of the Code Breached
- Incompetence
- Unethical Behaviour
Supporting documentation is needed in regards to each allegation.
Complaints regarding billing will not be reviewed or accepted as a valid complaint. Billings are between the Consumer & Member only and do not include the CBA.
